Our Allied Health Approach

At Hearth Allied Health we work with adults and children aged 7 years and older. We are a mobile service, based in Glen Iris. We will visit you in your home, school and community.  Our service can offer you a team approach to therapy. Our speech pathologists, occupational therapists and physiotherapists work together with you as a team.

Therapy services we offer:

  • Life skills
    • Increasing independence in personal activities including showering and dressing
    • Domestic activities including cooking and cleaning
    • Social and community participation including building relationships and increasing independence in the community
  • Aids and equipment
    • Eg. bed and mattress systems, mobility aids and seating, kitchen aids, bathroom aids, braces/supports, communication aids, etc.
  • Communication
    • Augmentative and alternative communication (AAC) devices and systems
    • Interaction and participation in your world 
    • Speech and language assessment and therapy

  • Home modifications
    • Ramps, bathrooms, kitchens, automation, etc.
  • Specialist Disability Accommodation / Supported Independent Living
    • Assessment and reports for your accommodation and support needs
  • Functional assessments
    • Functional assessments are used to help you identify goals you would like to work on.
  • Eating and drinking
    • Mealtime assessments
    • Mealtime profiles and training for support team
  • Exercise programs
    • Development of a program
    • Injury assessment, rehabilitation and management
    • Hydrotherapy, gym programs and Clinical Pilates
    • Education 
    • Accessing community programs 
    • Sports participation
  • Falls Assessment and Management
    • We assess your risk of falls and provide equipment and strategies to increase your safety at home and in the community
  • Education and support
    • For yourself, guardians/caregivers/support team to assist with achieving your therapy goals.

    The prices for all of Hearth’s services and supports in the NDIS are in line with the latest NDIS Pricing Arrangements and Price Limits.

    NDIS prices are reviewed annually to ensure that NDIS participants receive value for money and that service providers, like Hearth, can provide services in a sustainable market.

    When a new NDIS Pricing Arrangements and Price Limits is issued, the prices for all Hearth supports are adjusted in line with the new guide.

    Cancellation Policy

    You are also able to pay for Hearth Services privately if you are not eligible for the NDIS or wish to purchase additional service hours.

  • What geographical areas do you service?

    Our office is in Glen Iris. We are able to travel up to 30 minutes, as per the NDIS guidelines.

  • Do you work with children under 7 years of age?

    No, we are not a provider of early childhood intervention.

  • How do I make a referral?

    Please complete our Service Request form. 

  • What is the cost of your service?

    Our rates are in line with the NDIS Pricing Arrangements and Price Limits and can be downloaded here
